To complete and submit your Special Authorization form, please open the secure link from the original task email and select the blue Sign Special Authorization option. Opening this link will bring you to the task landing page, seen below.
Once here, select the blue Start button and you will be presented with three options: downloading your form, scanning your form to a PDF file, and uploading & submitting your form.
Please download the file using the Save Authorization Form link in the first panel. If you do not have a printer at home, please print the form at your nearest copy center, such as a Fedex, UPS, or public library, or reach out to your insurance representative directly.
Files must be in PDF format to be uploaded. If you attempt to upload a photo of the form, it will be rejected. The best option is to scan the completed form at your local copy center or using a smartphone device. For instructions on scanning your form with a smartphone device, select the blue How to scan documents with a smartphone link in the second panel.
When your file is ready to be uploaded, you can either drag and drop the file into the third panel of the task page or you can open the blue browse link to select it from your files. Once the file has been uploaded, the Submit button will turn blue. Press this button and you will be brought to a confirmation page.
Healthcare facilities often have stringent requirements for accepting Special Authorization forms. Use these tips to avoid rejection of your form:
Provide a handwritten signature - many facilities reject electronic signatures
Provide a witness signature if the form requests one
Provide a single PDF file, not an image file or a file for each page
Upload a high quality resolution file - facilities will reject blurry or illegible forms
The form is on a flat surface and all text is unobstructed by shadows or objects when scanned
Once your completed form has been uploaded, it will be reviewed by your healthcare facility. If you receive an additional task email to complete the exact same form, your form was likely rejected for not meeting one of the criteria listed above. If you do not receive an additional task notification, your form was accepted and no further action is required.
